COVID-19 and Safety FAQ

We strongly encourage you to download and read the DOE’s detailed Health and Safety Handbook. You can read this handbook above, or you can download a copy by clicking on tinyurl.com/2h7cffup.

Does my student have to complete a health screening form every day?

Yes.  All students must complete their health screening at home before coming to school (including a daily temperature check). Students will be able to complete paper forms daily (we have two attached to this year's back-to-school mailing).

 

We encourage all students to use the online form if possible to save paper. The online form can be completed on a cell phone or device each day at: healthscreening.schools.nyc. (If your child does not have a DOE account, select the “Guest” login and then when asked for the school you can check the box “other” and enter Queens Collegiate as the school.) The online form will show a green check after it is completed. Your student will need to show this check on their phone or device to enter the building if using the online form instead of the paper form.

​Does my student need to have the COVID vaccine to attend school?

No.  DOE and Queens Collegiate strongly recommend that all eligible students get vaccinated.  For more information on COVID vaccines in New York City, families can make appointments at vaccinefinder.nyc.gov or by calling 877-829-4692.   

 

Will my student be tested in school for COVID-19?

Maybe.  DOE is planning to test 10% of unvaccinated students twice monthly (every other week).  Families must give permission for COVID testing, which they can do using the paper form attached to this year's back-to-school mailing.

 

Do I have to give permission for COVID testing if I gave it last school year?

Yes.  DOE is requiring that all guardians of unvaccinated students submit a new form this year, regardless of last year.
